1. CONSENT TO PARTICIPATE
I, the undersigned parent/legal guardian of the above-named minor (“Participant”), hereby grant permission for my child to participate in filming, interviews, and related activities associated with the “Under the Stadium Lights” reality series, produced by IKONIK National Dance Network (“IKONIK”).

2. USE OF NAME, IMAGE & LIKENESS
I grant IKONIK and its affiliates the irrevocable right to film, photograph, record, and use the Participant’s name, image, likeness, voice, and performance in any and all media formats, including but not limited to:
Television
Streaming platforms (including YouTube)
Social media
Marketing, advertising, and promotional materials
This includes use of interview footage captured for the first episode and beyond, at IKONIK’s sole discretion.
3. NO GUARANTEE OF PARTICIPATION OR SCREEN TIME
I understand and agree that:
Submission of footage or participation in interviews does not guarantee that the Participant will appear in the series.
No specific amount of screen time is guaranteed.
No dancer placement in the final cast or series is guaranteed by signing this agreement.
Final casting, editing, and inclusion decisions are at the sole discretion of IKONIK and its production team.
4. PURPOSE OF RELEASE
I understand that this release is being executed primarily for marketing, promotional, and content creation purposes, including but not limited to:
Development and promotion of the “Under the Stadium Lights” series
Promotion of IKONIK programs, events, and NFL-related performances
Social media and digital content distribution
5. NFL PARTICIPATION ACKNOWLEDGMENT
I understand that the reality series is centered around participation in IKONIK’s NFL performance opportunities, and that securing a spot in an NFL event is a separate process with its own requirements, fees, and commitments.
6. COMPENSATION
I understand that:
The Participant will not receive compensation for participation in filming, interviews, or use of likeness.
Participation is voluntary and for exposure and opportunity purposes only.
7. RELEASE OF LIABILITY
I hereby release and hold harmless IKONIK National Dance Network, its owners, employees, affiliates, partners, and production team from any and all claims, demands, or causes of action arising out of or related to:
Participation in filming or interviews
Use of recorded content
Distribution of media in any format
8. EDITORIAL CONTROL
I acknowledge that IKONIK has full creative and editorial control over all content produced, including the right to edit, modify, or alter footage in any manner deemed appropriate.
9. AGREEMENT
By signing below, I confirm that:
I am the legal parent/guardian of the Participant
I have read and understand this agreement in full
I voluntarily agree to all terms outlined above
