DIGITAL PARTICIPATION AGREEMENT, LIABILITY WAIVER & MEDIA RELEASE

Hosted By: Solid Dance Battle LLC, a limited liability company, doing business as IKONIK Nationals and IKONIK Dance App (“IKONIK”)

Location: New York Jets Training Facility – Florham Park, New Jersey

By checking the box below, I acknowledge and agree to the following:

1. ASSUMPTION OF RISK

I understand that participation in dance workshops, rehearsals, auditions, and related athletic activities involves inherent risks including, but not limited to: Physical injury (minor or serious), sprains/strains/fractures, slips/falls/collisions, emotional stress or fatigue. I voluntarily assume all risks associated with participation in this event.

2. RELEASE OF LIABILITY

In consideration of being allowed to participate in the IKONIK Nationals New York Workshop & Audition hosted by Solid Dance Battle LLC d/b/a IKONIK, I hereby release, waive, discharge, and hold harmless Solid Dance Battle LLC, IKONIK Nationals, IKONIK Dance App, event staff/instructors/choreographers, independent contractors, venue partners (including the New York Jets Training Facility), and sponsors/affiliates/officers/members/agents/representatives from any and all claims, liabilities, damages, demands, causes of action, losses, or expenses arising out of or related to participation in this event, whether caused by negligence or otherwise, to the fullest extent permitted by law, including personal injury, illness, property damage, or loss.

3. MEDICAL AUTHORIZATION

I certify that the participant is physically fit to participate in dance and athletic activity. In the event of an emergency, I authorize Solid Dance Battle LLC d/b/a IKONIK to secure medical treatment for the participant if deemed necessary. I understand and agree that I am financially responsible for any and all medical expenses incurred.

4. MEDIA RELEASE (PHOTO, VIDEO & BROADCAST)

I grant Solid Dance Battle LLC d/b/a IKONIK, its affiliates, licensees, and assigns full permission to photograph/film/record video/capture audio/livestream portions of the event. I understand that media may be used for social media promotion, website content, YouTube and streaming platforms, marketing materials, advertising, future promotional campaigns, and streaming inside the IKONIK App. I waive any right to inspect or approve final media content and release all claims to compensation related to the use of these materials. This media release is granted in perpetuity, worldwide, and without limitation.

5. INSURANCE & PERSONAL PROPERTY

I understand that Solid Dance Battle LLC d/b/a IKONIK does not provide health, accident, or liability insurance coverage for participants. I am responsible for maintaining my own medical insurance coverage. IKONIK is not responsible for lost, stolen, or damaged personal property.

6. CODE OF CONDUCT

I agree that the participant will maintain respectful behavior, follow all staff instructions, refrain from unsafe or disruptive conduct, and adhere to all facility rules. Failure to comply may result in removal from the event without refund.

7. NO GUARANTEE OF PLACEMENT

I understand that participation in this workshop and audition does not guarantee selection, placement, performance opportunity, or team membership. All selection decisions are final.

8. ACKNOWLEDGMENT

By checking this box, I acknowledge that I am 18 years of age or older OR I am the legal parent/guardian of the participating minor; I have read and understand this agreement; I voluntarily agree to all terms and conditions stated above; and I understand that this is a legally binding agreement.
